The ALS/BLS Emergency Medical Service member evaluation form is completed online and sent to the RVFD Rescue Chief/Captain, Training Chief, and web-archive permanently. Once you have reached five competent precepts, your evaluations will be forwarded to the RVFD Medical Director for evaluation and approval. The approval process may take some time, so be sure to continue leading incidents and gaining additional evaluations. The MD may deem a submission unsatisfactory. A member must have five satisfactory evaluations to be approved to lead on their own with their medical license.
